Procedure for entering changes to a student's personal data in SIS

Data that a student can change about himself/herself in SIS

A student can change the details of the following information in the module Personal Data and Settings, on the Data change tab.

  • change the postal address to a contact address („other address“ in SIS), including the option to set up the delivery to a data box
  • email address (it is recommended to use a faculty email address)
  • mobile phone number and two other phone numbers if applicable
  • bank account number (Note: The bank account provided in the application form for these scholarships is used for the payment of the accommodation support or social scholarship)
  • the dormitory details, including the dormitory room number
  • URL of the student's website

Data that can only be changed by the Student Affairs Division (or exclusively by the Human Resources Division if the student is also employed by the Charles University)

The following is a list of the student's personal data, which can only be changed by the study department. In case the student is also employed by the University, the personal data may only be changed by the HR department of the faculty or another part of the University. The change may be made based on a written notification of the change of data and upon showing an ID card or passport or other document, if indicated for relevant data.

  • surname (family name), first name, middle name and maiden name (maiden name is proven by a civil registry document - e.g. marriage certificate)
  • marital status (proven by a civil registry document - e.g. marriage certificate)
  • degrees before and after the name - obtained by studying at the CU or at another school
  • birth number or birth code of the foreigner (this is not a health insurance code)
  • gender (automatically derived from the birth number)
  • nationality and citizenship qualifier (citizen, refugee, etc.)
  • Czech or Slovak ID card number
  • up to two passport numbers (for foreigners)
  • municipality, district and state of birth
  • information on whether or not the person is a permanent resident of the Czech Republic
  • permanent address
  • highest level of education attained
  • indication of altered working capacity - on presentation of relevant health certificates
  • indication of health limitation and any comment on health status - on presentation of relevant health certificates
