Submitting requests via the information system of Charles University

The request submitted via electronic infomration system are submitted in the module Admission of the Charles University Study Information System within the deadline specified in the Requirements for admissions procedure. The requests are submitted separately for each application for study.

1) In the module Admission go to My applications and click on the cross next to given application for study (the one for which you want to submit a request)

2) Use the link to Written Requests and click on the button Submit a written request.

3) Choose the type of request you need:

- Modification to the method of assigning subjects or the course of the entrance examination

In the text, specify what should the modification look like – e.g. bigger characters in the written text (15 points), longer time to complete the examination.

Choose the kind of hendicap and I agree that your personal data can be used for the university needs.

Attach a scan of a report from a medical or psychological institution and choose the attachment type: Modification of the entrance exam.

- Altermative date of the entrance examination 

The request can be submitted only after the exact date of the entrance exam is communicated, i.e. after receiving the invitation to the entrance examination. If there are more dates, choose a specific date.

For this request, you need to attach a doctor's confirmation of your sickness during the entrance exam, your school's confirmation about the date of your final exam (maturita exam, state exams etc.) or another attachment proving you were not able to attend the entrance examination. After attaching the file, choose the attachment type Alternative examination date.

- Recognition of Foreign Education

The request must be submitted by all applicants who graduated from high school/secondary school (in case of applicants for bachelor programmes) or from university (in case of applicants for master or doctoral programmes) outside Czechia.

All the documents (scanned proof of nostrification, or also scanned foreigner diploma and its supplement) must be submitted electronically as attachement type Copy of a secondary school leaving diploma or Copy of a university diploma. But the certified printed copies together with printed and signed copy of the electronic application also need to be sent to the Student Register, Albertov, Praha 2, or brought to the enrolment for the first year of study. Without that, the request will not be processed and the student will not be able to enrol for their studies.

- Other application concerning the admission procedure

This type of request is aimed solely at the applicants for doctoral studies who need to upload their request to take the entrance exam via telecommunication technologies (without being physically present at the faculty). This request must be sent by May 19, 2019.

4) After filling in all the obligatory fields and attaching the file (including the specification of the files' type), submit your request by clicking on Submit request.

The decision on the request will be sent via the Study Information System of the Charles University.
